Service Overview:












This service begins in the hospital when a healthcare team member identifies a need for additional post-discharge support. The hospital team will inform your GP and our pharmacy team, providing details about your discharge medications, reasons for prescriptions, and any changes made during your hospital stay.

At Ascent Pharmacy:












Upon receiving your discharge referral, our team reviews your discharge papers and compares them to your regular medications. We coordinate with your GP or hospital prescriber to resolve any issues quickly.

Initially, you will be discharged with a supply of medications, so you may not hear from us immediately. However, once your first prescription from your GP arrives, our team will contact you to check on your recovery.

Consultation:












During your consultation—either in the pharmacy or over the phone—you can discuss your medications, how you are managing them, and any concerns you may have. We will also help you dispose of any unwanted medicines.
